Psychiatric Evaluation: $300

  • Approximately 60-75 minutes in length

  • History of presenting problem

  • Psychiatric History

  • Medical History

  • Family History

  • Social History

  • Assessment/Diagnostic Impression

  • Treatment Plan Options

Follow Up Visit: $150

  • Approximately 30 minutes

  • Required every 1-3 months based on treatment needs

  • Evaluation of progress

  • Address side effects

  • Answer questions

  • Address subsequent issues
